Credit Repair Service Agreement
I, [CLIENT_FIRST_NAME] [CLIENT_LAST_NAME] hereby enter into the following agreement with Team USA Credit Repair.
- To evaluate Customer’s current credit reports as listed with applicable credit reporting agencies and to identify inaccurate, erroneous, false, or obsolete information. To advise Customer as to the necessary steps to be taken on the part of Customer in conjunction with Our Company, to dispute any inaccurate, erroneous, false or obsolete information contained in the customer’s credit reports.
- To prepare all necessary correspondence in dispute of inaccurate, erroneous, false, obsolete, and incomplete information in customer’s credit reports.
- To review credit profile status from the credit reporting agencies such as: Experian, Equifax, and Transunion. Consulting, coaching, and monitoring services are conducted by personal meetings, webinars, video conferencing, telephone, email, or by any other form of communication during normal business hours.
In exchange I, [CLIENT_FIRST_NAME] [CLIENT_LAST_NAME] agrees to pay Team USA Credit Repair an initial registration and setup fee of $199 to be billed at the initiation of services and authorize a monthly service fee of $150 to be automatically billed every 30 days thereafter until services are completed and or this agreement is terminated.
Authorization for Credit Repair Action
- (1)I, [CLIENT_FIRST_NAME] [CLIENT_LAST_NAME], hereafter known as “client” hereby authorize, Team USA Credit Repair, 2500 Southbridge Pkwy Suite 650, Birmingham, AL 35209 to make, receive, sign, endorse, execute, acknowledge, deliver, and possess such applications, correspondence, contracts, or agreements, as necessary to improve my credit. Such instruments in writing of whatever and nature shall only be effective for any or all of the three credit reporting agencies which are TransUnion, Experian, Equifax, and any other reporting agencies or creditor’s listed, as may be necessary or proper in the exercise of the rights and powers herein granted.
- This authorization may be revoked by the undersigned at any time by giving written notice to the party authorized herein. Any activity made prior to revocation in reliance upon this authorization shall not constitute a breach of rights of the client. If not earlier revoked, this authorization will automatically expire twelve months from the date of signature.
- The party named above to receive the information is not authorized to make any further release or disclosure of the information received. This authorization does not authorize the release or disclosure of any information except as provided herein.
- I grant to Team USA Credit Repair; 2500 Southbridge Pkwy Suite 650, Birmingham, AL 35209, authority to do, take, and perform, all acts and things whatsoever requisite, proper, or necessary to be done, in the exercise of repairing my credit with the three credit reporting agencies, which are TransUnion, Experian, Equifax and any other reporting agencies or creditor’s listed, as fully for all intents and purposes as I might or could do if personally present.
- I hereby release Team USA Credit Repair, 2500 Southbridge Pkwy Suite 650, Birmingham, AL 35209 , from all and all matters of actions, causes of action, suits, proceedings, debts, dues, contracts, judgments, damages, claims, and demands whatsoever in law or equity, for or by reason of any matter, cause, or thing whatsoever as based on the circumstances of this contract.
(A) This Credit Repair Service Agreement between Team USA Credit Repair and the undersigned “Client” is for the purpose of purchasing credit restoration services (The “Services”). The Services will include preparation of correspondence to the Credit Bureaus requesting removal of errors, misrepresentations, unverifiable information, which the Client states appear on their credit reports which the Client has furnished Team USA Credit Repair. This is not a Debt Consolidation or Bill Payment Program. Federal Law requires that any unverifiable, outdated or erroneous information must be removed from consumer credit reports by Reporting Agencies. Team USA Credit Repair agrees to use its best efforts to provide the Services and will perform them in accordance with Federal and State Laws.
(B) Team USA Credit Repair will perform an audit on the Client’s credit reports and develop an action plan to delete, correct or change inaccurate, incomplete, unverifiable, and obsolete items under current Federal and State laws during this contract. An initial registration and setup fee of $199 to be billed at the initiation of services and a monthly service fee of $150 to be automatically billed every 30 days thereafter until this agreement has been terminated. Client understands and agrees that after a month of services has been fully performed, remaining fees will be spread out and will be due and that this fee is for all costs and fees associated with the previous month’s Services. This process will continue until the end of the service term. The Client understands that the remaining payments fees include the continuing analysis/audit and restoration Services. All correspondence associated with the credit improvement process, the review for changes requested by the Client to the Client’s credit reports as a result of contacts made on the Client’s behalf with each applicable Credit Bureau, Creditor or Public Record Holder, phone consultations with Client and the continuing planning and creation of documents for the purpose of credit restoration.
(C) Non-Payment. If any form of payment you supply is uncollectible for any reason, Team USA Credit Repair may charge you a dishonored payment fee of $30.00. The Client will agree not to close the bank account or credit card that Team USA Credit Repair is authorized to withdraw payments from. Should the Client be required to change the authorized bank account or credit card, you must contact Team USA Credit Repair immediately and change their payment method and authorization to withdraw funds through our payment system, as any interruption in the payment of our fees will require Team USA Credit Repair to discontinue your service agreement. The resulting actions undertaken by the credit card companies, collection agencies, and/or law firms against the client will not be the responsibility of Team USA Credit Repair. Client also agrees that they will not request a chargeback for any payments made to Team USA Credit Repair for services already performed with their bank or credit card company. Client understand such actions will result in the immediate termination of services and that a claim will be handed over to a collection agency for the amount of the chargeback and any associated fees, that the collection will be reported to the credit bureaus, and that legal action could be brought against them.
(D) Credit Application. The client shall not apply for any type of credit until they have completed the process unless otherwise recommended by Team USA Credit Repair. If the client fails to comply with our expressed request and applies for any credit and is denied, we cannot be held responsible for additional negative remarks and the direct influence this might have on your credit score.
(E) Client agrees not to miss or be late with any payments on any of their bills, and not make major purchases on credit cards or increasing balances on existing credit cards unless absolutely necessary. Client agrees to keep their credit monitoring account active until services are completed and or this agreement is terminated. Client agrees to send, via upload from your portal provided by Team USA Credit Repair, all credit reports and/or correspondence received from credit bureaus and/or creditors to Team USA Credit Repair within five (5) days after the date received to guarantee the success of this program. If the Client has not received any credit reports or correspondence from the credit bureaus within 60 days after the date of the initiation of services, Client must notify Team USA Credit Repair so appropriate measures can be taken. Noncompliance can result in termination of the account.
(F) By executing this Contract to obtain Team USA Credit Repair Services, Client grants Team USA Credit Repair during the term of this Contract, a limited power of attorney, by and through its authorized representatives, to: (1) Use the Customer Information that the Client provides in order to obtain from credit bureaus, creditors, collection agencies and other holders of records of Client’s credit reports, Client’s credit history or other creditor information for the Services; (2) Sign correspondence to the record holders; (3) Use Client’s name to sign correspondence addressed to creditors; (4) Obtain credit information over the telephone, fax, and or through the internet from record-holders; (5) To discuss information with any record-holders to help resolve a debt if mediation of a debt is necessary.
(G) Team USA Credit Repair acknowledges that its Authorized Representatives have been alerted to the sensitivity of the Customer Information. As such, Team USA Credit Repair will use its best efforts to ensure that Customer Information will be handled in a responsible and professional manner. The Customer shall have the right to revoke or terminate the limited power of attorney provided under this Contract at any time upon written notice to Team USA Credit Repair. Otherwise, the limited power of attorney shall terminate upon termination of this Contract. All questions pertaining to validity, interpretation, and administration of this Contract shall be determined in accordance with the laws of the State of Alabama. Client agrees that Client’s limited power of attorney is valid throughout the United States for all Customer Information to be obtained by Team USA Credit Repair pursuant to this Contract by the binding and enforceable signatures set forth below. This Agreement contains the entire agreement of the parties and there are no other promises or conditions in any other agreement whether oral or written. This Agreement supersedes any prior written or oral agreements between the parties.
(H) Money Back Guarantee/Cancellation. This agreement may be canceled by either party without any penalty or further obligation at any time. If after 90 days Team USA Credit Repair has not been able to improve your credit file by removing any derogatory items from your reports and you do not feel as though we worked your case with maximum effort and diligence, we will gladly offer you a full refund of this service, as long as the client has followed instructions to not allow companies to pull your credit unnecessarily, not applying for new credit unless otherwise recommended to do so, not missing or being late with any payments on any of their bills, and not making major purchases on credit cards or increasing balances on existing credit cards unless absolutely necessary, and to keep their credit monitoring account active until services are completed and or this agreement is terminated. Client agrees to send, via upload from their portal, all credit reports and/or correspondence received from credit bureaus and/ Team USA Credit Repair agrees to improve client(s) credit profile of inaccurate, unverifiable and incomplete items during the term of this agreement. Client(s) understands that the results obtained by Team USA Credit Repair on behalf of client(s) are dependent on numerous factors, including but not limited to client(s) ability to repay debts and loans, cooperation of client(s) creditors, and credit bureaus ability to verify information provided to them. Client agrees to only communicate with the credit bureaus through Team USA Credit Repair written correspondence.
(I) I, the client, understand that with proper information I could undertake the same or similar techniques to repair my own credit as choosing to hire Team USA Credit Repair to undertake the services outlined in this agreement without duress or provocation. Client agrees to hold Team USA Credit Repair and its employees, officers, directors, agents and representatives harmless from any claim, suit action or demand made by any of my creditors or any other person which may arise from the action(s) taken by my creditors in connection with any services rendered by Team USA Credit Repair on my behalf. In the event Team USA Credit Repair engages in collection efforts, Client will be required to reimburse Team USA Credit Repair for out-of-pocket expenses as the result of such efforts.
Consumer Credit File Rights Under State and Federal Law
You have a right to dispute inaccurate information in your credit report by contacting the credit bureau directly. However, neither you nor a credit repair company or credit repair organization has the right to have accurate, current and verifiable information removed from your credit report. The credit bureau must remove accurate, negative information from your report only if it is over 7 years old. Bankruptcy information can be reported for up to 10 years.
You have a right to obtain a copy of your credit report from a credit bureau. You may be charged a reasonable fee. There is no fee, however, if you have been turned down for credit, employment, insurance, or a rental dwelling because of information in your credit report within the preceding 60 days. The credit bureau must provide someone to help you interpret the information in your credit file. You are entitled to receive a free copy of your credit report if you are unemployed and intend to apply for employment in the next 60 days, if you are a recipient of public welfare assistance, or if you have reason to believe that there is inaccurate information in your credit report due to fraud.
You have a right to sue a credit repair organization that violated the Credit Repair Organization Act. This law prohibits deceptive practices by credit repair organizations.
You have the right to cancel your contract with any credit repair organization for any reason within 3 business days from the date you signed it.
Credit bureaus are required to follow reasonable procedures to ensure that the information they report is accurate. However, mistakes may occur.
You may, on your own, notify a credit bureau in writing that you dispute that accuracy of information in your credit file. The credit bureau must then reinvestigate and modify or remove inaccurate or incomplete information.
The credit bureau may not charge any fee for this service. Any pertinent information and copies of all documents you have concerning an error should be given to the credit bureau.
If the credit bureau’s reinvestigation does not resolve the dispute to your satisfaction, you may send a brief statement to the credit bureau to be kept in your file, explaining why you think the record is inaccurate. The credit bureau must include a summary of your statement about disputed information with any report it issues about you.

You may cancel this contract without penalty or obligation at any time before midnight of the 3rd business day after the date on which you signed the contract.
